New Patient Registration
Patient Information Leaflet
If you decide to register as a patient with us you need to do is come to Reception and complete a form. We no longer have the facility for patients to register with us online as the process was not fail-safe. If you are on repeat medication, you should bring your repeat slip with you when you next request medication or have your first appointment, giving at least 48 hours’ notice, excluding weekends and bank holidays.
All patients registered with the Acorn Surgery will have a named GP. Patients can request a change of named GP and when making an appointment, can see any of our clinical team and rather than a specific doctor.
New patients with an existing medical condition or who are on medication are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
You will need to show ID upon registering at the Practice if you wish to have access to the Patient Online system to access coded data in your medical record.
Changes to Personal Details
If you change your name, address, home or mobile telephone number or email address, particularly if your number is ex-directory, please inform us as soon as possible. It may be important for us to contact you at home or work.
